Form Name

The name that gets emailed to you. If you have multiple forms on your site, differentiate them with a descriptive form name. This name does not appear on the website.

Pass this as a default URL Parameter

This is used in the page code on the page where the contact form appears. It indicates that this specific form should appear on the website on that specific page.

Sent to Email

The email that the form is sent to. It can be multiple emails. If this field is left blank, the form will be sent to the email set as the Admin email in the CMS Settings module.

Subject

This will appear in the subject line of the email the form is sent to.

Automatic Response Subject and Email

If you would like a response sent to the person who filled out the form, Automatic Response Active must be set to Yes. You can then create any message you think is suitable.
